Umbridge was raped by the centaurs.
In Greek mythology a main characteristic for centaurs is that they take away human women and rape them. JK Rowling knows that.
The reaction of Umbridge afterwards tells us that she had been traumatised but seemed to not have been physically hurt.
JK Rowling has, before and later, written about lots of adult things and dark topics in the books, including rape. Implying rape would not be untypical.
The centaurs in Harry Potter universe is proud, feel superior and hate humans, and would never rape humans.
JK Rowling would never write about rape in that way, and justify rape as a punishment for a bad character.
Hermione led Umbridge to the centaurs and didn't stop them from dragging her away. If she knew that they would rape Umbridge, she would never do that.
